What to Expect of the Admissions Liaison

As an admissions liaison, my main priority is to make sure you as the patient or loved one have all of the information you need to make an informed decision when picking a skilled nursing home for long term or short term care. Our admissions liaison will gladly visit the facility or hospital in which you or your loved one is currently residing one the information is received with your permission from your social worker.

Questions to Ask

admissions liaisonWhen our admission liaison visits you or your loved one, you should be prepared to ask the right questions about the facility to ease your mind of any concerns with the transition in care. When an admissions liaison comes to visit you or your loved one, questions such as discharge information after treatment, long term plans, home medications, if there is any special medical equipment needed, and discuss insurance or payment information. The more information we are given before admission to our facility, the better we can care for you or your loved one.

Some of the questions you should ask are questions such as insurance length of coverage, room information, visitation, future appointment transportation, location if unknown, what you can expect from us for therapy services, and any other details that are most important to you or your loved one’s stay with Abbyshire Place.
